Selecting the best course of intervention for patients with endometriosis and fertility concerns can be challenging, especially for those with ovarian endometrioma. While surgery is often the preferred option for severe patients, many traditional tools risk compromising the ovarian reserve and as a result, patients may delay surgical treatment.
A recent study, published in Human Reproduction Journal, describes CO2 Laser vaporization, according to the ‘three-step procedure’, has been proposed as the best method to preserve ovarian function.
This study, published in October 2018, presents a multi-center randomized clinical trial including 60 patients, comparing vaporization using CO2 laser (UltraPulse Duo system, Lumenis) to the current standard treatment option - laparoscopic stripping - for benign cysts.
Study results were:
• Patients in CO2 laser vaporization group had a significant increase of AFC
• AMH levels were significantly reduced at 3 months in the cystectomy group compared with no reduction in the CO2 laser vaporization group
The authors conclusion was that CO2 technology may treat endometrioma with minimal damage to the adjacent healthy ovarian tissue, assisting in preserving fertility.
