Vaccination Proof for Indoor Activities (Key to NYC)

Pursuant to the Emergency Executive Order No. 226 dated August 20, 2021, issued by the City of New York – Office of the Mayor, “Key to NYC” requires people 12 and older to provide proof of vaccination (i.e., receipt of at least one dose of an approved COVID-19 vaccine) to access certain on-site amenities including dining, banquet, and fitness facilities.

Generally, the proof of vaccination requirements applies to all employees, patrons, interns, contractors, volunteers, and event vendors (including, without limitation, audio visual professionals, entertainers, and outside caterers) aged 12 and over.

Valid proof of vaccination status may be in the form of:
  • CDC Vaccination Card – a photo or photocopy is acceptable
  • NYC Vaccination Record or other official immunization record, including from your health care provider. A digital photo or photocopy of this card is also acceptable. If you are unable to get this record, call 311.
  • NYC COVID Safe App: Android | iOS.
  • CLEAR Health PassAndroid | iOS. You can use the digital vaccine card option in the CLEAR app if you are 18 and older and are fully vaccinated.
  • Excelsior Pass App
  • A photo or hard copy of an official vaccination record of a vaccine administered outside of the U.S. for one of the following for one of the following vaccines: AstraZeneca/SK Bioscience, Serum Institute of India/COVISHIELD and Vaxzevria, Sinopharm, or Sinovac (the foregoing approved list is current as of August 14, 2021)

Proof of vaccination for vaccines administered outside the U.S. may be demonstrated by an official immunization record that includes all the following:
  • First name and last name
  • Date of birth
  • Vaccine product name (ex: AstraZeneca/SK Bioscience)
  • Date(s) administered
  • Site where the vaccine was administered or person who administered the vaccine
      • For people vaccinated outside of the U.S., the country of origin may be listed as the site where the vaccine was administered


Identification bearing the same identifying information as the proof of vaccination. The identification must confirm that the person requesting entrance is the same person reflected on the proof of vaccination. Accordingly, the identification must contain either:

  • The name of the individual and a photo of the individual; or
  • The name of the individual and the individual’s date of birth.

Examples of acceptable forms of identification include but are not limited to:

  • Driver’s license
  • Non-driver government ID card
  • IDNYC card
  • Passport
  • School or work ID card
  • Copies of identification document, including a picture on a mobile device or by using an app like NYC Covid
    Safe that allows the individual to display a copy of the document


Note: People who have proof they received an active COVID-19 vaccine during a U.S.-based clinical trial are also permitted at the above activities.

Discrimination Complaints and Reasonable Accommodations

Indoor dining, fitness and entertainment and meeting spaces must accept valid proof that you received at least one dose of any vaccine authorized for emergency use by the FDA or WHO. This requirement may not be used as an excuse for businesses to discriminate against anyone who has valid vaccination proof.
